Health System Vendor Credentialing Guide
Mayo Clinic uses Symplr for vendor credentialing. This guide covers registration, required documents, and how to get approved to work in Mayo Clinic facilities.
Log In to Symplr → Symplr Full GuideFollow these steps to register through Symplr and gain access to Mayo Clinic facilities.
Go to app.symplr.com and register as a new vendor representative. Use your company email address.
Once logged in, search for "Mayo Clinic" and select the specific facility or campus you need access to. Each location may have separate requirements.
Upload all required documentation (see checklist below). Documents must be current and match your profile information exactly.
Complete the annual Symplr credentialing fee payment. Typical range is $150–$295/year. This fee is separate from any Mayo Clinic charges.
Mayo Clinic will review your submission. Processing typically takes 5–15 business days. You’ll receive an email from Symplr once approved.
Once credentialed, use the Symplr mobile app or kiosk to check in and out each time you visit a Mayo Clinic facility. Badge required on-site.
Requirements vary by facility and product type. Always verify current requirements in Symplr.

Current COI naming Mayo Clinic. Minimum $1M per occurrence typically required.
Flu shot (seasonal), MMR, Varicella, Hepatitis B series, Tdap, TB test/IGRA.
Criminal background screening completed through Symplr-approved provider.
10-panel drug test from an approved collection site. Results uploaded to Symplr.
Annual HIPAA compliance training certificate. Many vendors complete via Symplr’s training portal.
OIG and SAM.gov exclusion verification. Completed automatically through Symplr in most cases.
Credentialing is managed through Symplr for all major campuses. Select the correct facility when registering.
